For years the standard advice after a concussion was simple: go home, rest in a dark room, and wait it out. A run of research published over the past year suggests that the waiting may itself be part of the problem.

Reading through the concussion studies released in early 2025, one theme came up again and again. People who are assessed and start rehabilitation sooner tend to recover faster.

The numbers are striking. In one study of children and teenagers, those seen within three days of injury recovered in around 32 days on average. Those who waited more than a week took over 56 days, close to twice as long. Every extra day before assessment was linked to a noticeably greater chance of a drawn-out recovery stretching past a month.

A randomised trial in adults found the same pattern: people who began physiotherapy earlier improved more quickly than those who started later. A Canadian study of adolescent athletes found that both early and late presenters got there in the end, but the early group got there sooner.

Understandably, people worry that doing something too soon might make things worse. A large trial across 28 sites tested exactly that. Athletes who began gentle, targeted activity while still having mild symptoms had very few flare-ups, and nearly all of those settled by the next session. Starting carefully and early appears to be safe.

None of this means pushing through pain. It means being guided, having someone check your balance, vision, neck and tolerance for activity, then building a plan around what they find.
